The Federal Government of Nigeria has approved a replacement policy for subscribers whose Subscribers Identification Module (SIM) may have been lost, stolen, misplaced or damaged.

Dr. Ikechukwu Adinde, Director, Public Affairs, Nigerian Communications Commission (NCC), and Mr Kayode Adegoke of the National Identity Management Commission (NIMC), jointly made this known in a statement on Saturday in Abuja.

The newly approved SIM Replacement requirements are as below.

Regarding SIM Replacement (Swap) Requirements, What Has Changed?

a)  NIN is now compulsory for All SIM Replacement (Swap) Types.

b)  Subscribers should come along with their NIN Slip; This will also serve as their ID.

c)  All other SIM Replacement (Swap) Requirements remains valid!

For SIM Replacement – Upgrade

There is still a temporary waiver for “No SIM Pack/Affidavit for SIM Upgrade” for another Six (6) months effective December 7, 2020.

1. Customer NIN Slip/Identification (ID)  

2. Completed SIM Swap Form

3. Customer Live SIM (Active SIM)

4. Send OTP to Active Line/MSISDN with Customer (Ensure customer receive OTP in front of you).

5. Capture Customer Live Image & Upload (customer in front of you, do not capture on plain background).

6. Capture or scan All Documents & Upload.

7. Submit SIM Replacement (Upgrade) Request.

8. Proxy is not applicable for SIM Upgrade – OTP on Live SIM is Required for Upgrade

For SIM Replacement – SIM Change (lost/stolen/damaged):-

1. Customer NIN Slip/Identification (ID)

2. Customer Proof of Ownership:- SIM Pack or Damaged SIM with Serial (for damage SIM only).

3. Request sworn affidavit (if no SIM Pack or damaged SIM)

4. Completed SIM Swap Form

5. Validate Form Details (use eValidator).

6. Capture Customer Live Image & Upload (customer in front of you, do not capture on plain background).

7. Capture or scan All Documents & Upload.

8. Submit SIM Replacement (swap) Request.

For Proxy SIM Replacement – SIM Change:-

1. Customer & Proxy NIN Slip/Identification (ID)

2. Customer Proof of Ownership:- SIM pack or Damaged SIM with Serial (for damage SIM only).

3. Sworn Affidavit (Affidavit must capture customer name/MSISDN/reason for swap and reason for unavailability)

4. Completed SIM Swap Form (with Proxy details too)

5. Validate Form Details (use eValidator).

6. Capture Proxy Live Image & Upload (Image of Proxy; do not capture on plain background).

7. Capture or scan All Documents & Upload.

8. Submit SIM Replacement (swap) Request.

Important Note!

  • All Eyeballing Process remains valid (check form, ID, SIM pack, Live Image tallies with SIM Reg. Image etc.).
  • ID – Mandatory use of NIN Slip/ID as Means of Identification (No Affidavit for ID at this time).
  • SIM Pack – Customers without SIM Pack requires Sworn Affidavit (attesting to ownership and loss of pack).
  • Proxy SIM Swap customers require sworn affidavit (reason for swap & unavailability must be mentioned).
  • Proxy ID and Owner NIN must be submitted _ NIN Slip for Proxy and Owner.
  • Affidavits can be obtained from the Court or issued by a Notary Public.
  • Affidavits must have a Passport Photo, Court Stamp or Notary Public Seal.
  • Affidavits must mention the Customer Name, Line/MSISDN, Address etc.
  • Capture & Upload the right documents (ID, Form, SIM Pack/Affidavit, Customer Image etc.).
  • All Documents & Image captured for SIM Replacements (Swap) must be Clear.
